*PSI’s End User License Agreement does not allow for the contract in its entirety to be transmitted electronically (by email). However, PSI does grant permission for pages 1 (Client Information) and individual Pet Profile Sheets to be shared with your clients via email if you prefer to receive this information prior to the initial meeting. For your convenience, copies of the pages that can be emailed have been placed in a separate folder within the ZIP file you will download labeled “Portions that can be emailed.” Under no circumstances should any portion of the Forms be posted publicly online, but the contract can be uploaded to an online document repository for the purpose of review and signature by your clients, per the EULA. Read the complete End User's License Agreement (EULA) before purchasing.
